Grewal opened a statewide investigation into the high number of deaths at long-term care facilities during the pandemic. The investigation will determine whether any individuals or entities broke the law and should face civil or criminal penalties for their actions, or lack of action. Those with firsthand knowledge of illegal activity or other misconduct involving a New Jersey-based long-term care facility during the pandemic are asked to complete a form online at bit.ly/3dCWmWX. Documents may be uploaded through the form. You may remain anonymous, but the more detailed the submissions, the more helpful they will be for investigators.
